Switching from Wegovy to Zepbound

Why people look up Wegovy to Zepbound

Searchers often hope tirzepatide will restart weight loss after a plateau on semaglutide, or they have heard SURMOUNT-5 averages. Others need Zepbound's OSA indication. Others cannot fill Wegovy. Those are different clinical problems. A plateau has many causes (calories, muscle loss, sleep, other medicines, dose not yet at labeled maintenance). Switching without reviewing those factors can disappoint.

Match the new product to the actual indication

Wegovy's cardiovascular-risk indication in adults with established CVD is supported by SELECT. Zepbound does not replace that labeled claim. Conversely, Zepbound is labeled for moderate to severe OSA in adults with obesity; Wegovy is not a substitute OSA indication. Chronic weight management overlaps, but the PI language is still product-specific.

If the person also has type 2 diabetes, neither Wegovy nor Zepbound is Mounjaro or Ozempic. Glucose plans must be explicit.

SURMOUNT-5 in one careful paragraph

Adults with obesity without diabetes were assigned to maximum tolerated tirzepatide (10 or 15 mg) or maximum tolerated semaglutide (1.7 or 2.4 mg) for 72 weeks. Mean weight change was about -20.2% versus -13.7%. Waist circumference also fell more with tirzepatide. The trial was open-label. Gastrointestinal adverse events were common in both arms and mostly mild to moderate during escalation.

Use those numbers as context, not as a personal forecast after years on Wegovy. The trial did not enroll a 'switch from stable 2.4 mg' cohort as its primary design.

Timing: semaglutide hangs around

Weekly semaglutide has a long half-life (on the order of one week, with several weeks to wash out). Starting Zepbound 2.5 mg while a large amount of semaglutide remains may still be what a clinician chooses, or they may wait. Starting Zepbound 15 mg immediately after Wegovy 2.4 mg stacks two escalation problems.

The Zepbound label's default is 2.5 mg weekly for four weeks, then 5 mg, then 2.5 mg increments. Prior GLP-1 exposure is a fact the prescriber uses. It does not rewrite the maximum 15 mg weekly cap or the OSA versus weight maintenance targets.

Safety overlap

Both medicines: boxed warning for rodent thyroid C-cell tumors; contraindicated in MTC and MEN 2; pancreatitis and gallbladder warnings; volume-depletion kidney injury; not recommended in severe gastroparesis on current tirzepatide and related GLP-1 labels; aspiration risk with anesthesia; pregnancy concerns.

Do not combine full doses of both in the same week without explicit instructions. Do not share pens. Do not buy either product from a site that skips a licensed clinician.
