Wegovy dosing: weekly injection and tablet schedules

Wegovy injection titration chart

Wegovy injection is semaglutide given once weekly in the abdomen, thigh, or upper arm, with or without meals. The same-day-each-week habit matters more than the clock time. This chart is from the U.S. prescribing information and is educational. It is not a prescription. Beema Health does not dispense Wegovy.

If a step causes hard-to-tolerate nausea, vomiting, or other effects, the label allows delaying escalation for four weeks. That is still clinician-directed titration, not an unofficial microdose plan.

  • Weeks 1-4: 0.25 mg once weekly
  • Weeks 5-8: 0.5 mg once weekly
  • Weeks 9-12: 1 mg once weekly
  • Weeks 13-16: 1.7 mg once weekly
  • Week 17 onward: maintenance per indication (often 2.4 mg weekly recommended; 1.7 mg is an option)

Maintenance dose depends on the indication

Adult weight reduction: 1.7 mg or 2.4 mg weekly (2.4 mg recommended). After at least four weeks on 2.4 mg, the current label allows a maximum of 7.2 mg weekly in adults when additional weight reduction is clinically indicated and 2.4 mg is tolerated.

Adult cardiovascular risk reduction (established CVD plus overweight or obesity): 2.4 mg recommended or 1.7 mg weekly.

Pediatric weight reduction (ages 12 years and older meeting BMI criteria): 2.4 mg recommended or 1.7 mg weekly. The 7.2 mg adult option is not the pediatric schedule.

Noncirrhotic MASH with moderate to advanced liver fibrosis in adults: 2.4 mg weekly recommended; 1.7 mg if 2.4 mg is not tolerated, with possible return to 2.4 mg. Always use the latest label rather than memorizing an old blog table.

Wegovy tablet dosing

Oral Wegovy is taken once daily in the morning on an empty stomach with up to 4 ounces of water, swallowed whole, then wait 30 minutes before food, other drinks, or other oral medicines. Labeled tablet steps are 1.5 mg (days 1-30), 4 mg (days 31-60), 9 mg (days 61-90), and 25 mg daily thereafter.

The label describes switching between 2.4 mg injection and 25 mg tablets with specific washout timing. If 25 mg tablets are not tolerated, consider 1.7 mg injection. Details belong in a clinic visit, not in a DIY calendar app.

Administration, storage, and missed doses

Use a new needle when the device requires one. Never share pens. Rotate injection sites. Storage (refrigerator versus room-temperature windows) is in the Instructions for Use and in the travel-with-Wegovy article.

Missed-dose rules for the injection typically hinge on whether the next weekly dose is more than two days away. Taking two injections close together increases gastrointestinal risk. If several weeks are missed, the prescriber may restart titration rather than resume at 2.4 mg. Ask; do not guess.

When dosing should pause for safety

Hold and call a clinician for suspected pancreatitis, a serious allergic reaction, or severe dehydrating gastrointestinal illness. People with type 2 diabetes who also use insulin or a sulfonylurea need a hypoglycemia plan when Wegovy is added. Tell anesthesia teams before procedures.

Wegovy has a boxed warning for rodent thyroid C-cell tumors and is contraindicated in MTC and MEN 2. Pregnancy is a reason to stop. None of those issues is solved by 'just using a lower pen' without medical advice.

Why 2.4 mg is the number people quote from trials

STEP 1 and SELECT used 2.4 mg weekly after escalation. Mean weight change in STEP 1 was about -14.9% versus -2.4% with placebo at 68 weeks. SELECT showed a reduction in major adverse cardiovascular events in a high-risk population without diabetes. Quoting those averages is not a guarantee that 2.4 mg will produce the same change in one person, and it is not a reason to skip titration.

The current label describes a possible increase to 7.2 mg weekly in selected adults who already tolerate 2.4 mg when extra weight reduction is clinically indicated. Tablet maintenance is 25 mg orally once daily after a 90-day escalation.
What if I miss a Wegovy injection?
Follow the current Medication Guide. In general, if the next scheduled dose is more than 48 hours away, take the missed dose as soon as possible; if the next dose is due within 48 hours, skip the missed dose. Do not take two doses within 48 hours. Confirm on the label in your box, because instructions can be updated.

Can I start at 2.4 mg to lose weight faster?
No. Starting at the maintenance dose skips the labeled strategy for reducing gastrointestinal adverse reactions. Faster is not safer. Only a prescriber should change the schedule.
How is tablet dosing different from the injection?
Tablets are daily, not weekly, and must be taken fasting with a small amount of water. Escalation is 1.5 mg, then 4 mg, then 9 mg, then 25 mg, each for 30 days. Do not convert injection milligrams to tablet milligrams by arithmetic.
