ATTAIN-1 in plain language
The Phase 3 ATTAIN-1 trial, published in the New England Journal of Medicine in 2025, enrolled adults with obesity who did not have diabetes. Participants received 6 mg, 12 mg, or 36 mg of orforglipron, or placebo, for 72 weeks, plus guidance on diet and activity. All three doses beat placebo on percent weight change.
On the treatment-regimen (intention-to-treat) estimand, mean change was about 7.5% at 6 mg and 8.4% at 12 mg. Lilly later highlighted a 12.4% mean reduction (about 27 pounds) at the highest dose among people who remained on treatment, versus 0.9% with placebo. Blood pressure, triglycerides, and non-HDL cholesterol also improved versus placebo in the published trial. Discontinuation because of adverse events ranged from about 5.3% to 10.3% on drug versus 2.7% on placebo. Most problems were gastrointestinal and mild to moderate.
Those figures sit below the mean percent loss reported in STEP 1 for weekly semaglutide 2.4 mg (about 14.9% at 68 weeks) and well below SURMOUNT-1 means for tirzepatide. Cross-trial comparisons are imperfect because populations, diet instructions, and estimands differ. They are still useful context: an oral option can be clinically meaningful without matching the largest injectable trial averages.
How orforglipron differs from weekly GLP-1 injections
Approved injectable GLP-1-class medicines for weight management are typically taken once weekly. Orforglipron is daily. Daily dosing can help people who want a pill routine. It can also make missed doses a weekly problem instead of a once-a-week event. Foundayo's labeled instructions, not this article, govern how a prescribed patient should take it.
Because orforglipron is not a peptide, it is absorbed differently from semaglutide tablets. Rybelsus (oral semaglutide) has strict fasting and water rules. Lilly's Foundayo materials emphasize fewer timing restrictions. That convenience is real for some patients and irrelevant for others who already tolerate weekly injections.

Side effects and class warnings

Rare class risks discussed on injectable labels include pancreatitis, gallbladder disease, and the boxed warning about thyroid C-cell tumors in rodents. Do not assume a pill is gentler than an injection. Oral delivery still activates the GLP-1 receptor. People with a personal or family history of medullary thyroid carcinoma or MEN 2, a history of pancreatitis, or pregnancy plans need individualized counseling.
