Orforglipron versus semaglutide: route, data, and who offers what

Compare oral orforglipron (Foundayo) with injectable semaglutide using ATTAIN-1 and STEP 1. Beema Health does not offer orforglipron.

Same receptor family, different chemistry and schedule

Both orforglipron and semaglutide activate the GLP-1 receptor. Semaglutide is a peptide, usually injected weekly at obesity doses (2.4 mg in STEP 1 / Wegovy). Orforglipron is a non-peptide small molecule taken by mouth daily. In April 2026 the FDA approved orforglipron as Foundayo for chronic weight management in eligible adults. Semaglutide already had years of FDA-approved branded use.

ATTAIN-1 beside STEP 1, with caveats

STEP 1 used a reduced-calorie diet plus activity with weekly semaglutide 2.4 mg for 68 weeks in adults without diabetes. Mean weight change was about 14.9% versus 2.4% with placebo. ATTAIN-1 used healthy diet and activity (Lilly notes it was not a strict reduced-calorie protocol) with daily orforglipron for 72 weeks. Mean changes versus placebo were significant at all doses; they were smaller than STEP 1's headline mean.

If your only metric is 'largest published obesity-trial average,' weekly semaglutide 2.4 mg still sits higher. If your metric is 'FDA-approved daily tablet without injection,' orforglipron now exists as Foundayo. Those metrics answer different questions.

Daily adherence versus weekly adherence

Pills fail when days are skipped. Weekly injections fail when someone never learns the pen. Travel, shift work, and swallowing difficulties all matter. Orforglipron's lack of Rybelsus-style fasting rules is a genuine convenience point Lilly emphasized at approval. Convenience still does not erase gastrointestinal class effects.

  • Orforglipron: daily tablet, small molecule, Foundayo (FDA-approved 2026), not offered by Beema
  • Semaglutide obesity dose: weekly injection in STEP 1 / Wegovy, larger mean percent loss in that trial

Shared class risks

Nausea, vomiting, diarrhea, and constipation appear in both programs. Discontinuation for adverse events in ATTAIN-1 was about 5.3% to 10.3% on orforglipron versus 2.7% on placebo. Semaglutide labels discuss gallbladder disease, pancreatitis warnings, pregnancy, and the thyroid C-cell boxed warning. Access is not the same as a Beema cart

Foundayo is a branded product distributed through channels Lilly described at launch, including retail pharmacies. Beema's telehealth intake does not dispense it. If you want Beema's cash-pay compounded semaglutide, that is a different product with a different regulatory status: not FDA-approved, used only when legally available and clinically appropriate.

People shopping 'orforglipron research peptide' are in the wrong aisle. Orforglipron is not a peptide. Unverified capsules are not Foundayo.

How a clinician might choose

A licensed provider may weigh injection willingness, prior GLP-1 exposure, diabetes status, cardiovascular history (semaglutide 2.4 mg has SELECT data), insurance or cash-pay realities, and tolerability. This article will not pick for them. If you start Beema intake, list every GLP-1 you have tried, including pills, so the reviewer is not flying blind. A prescription is never guaranteed.

Is orforglipron stronger than semaglutide?
Published obesity-trial means do not show that. STEP 1 reported about 14.9% mean weight change with weekly semaglutide 2.4 mg at 68 weeks. ATTAIN-1's treatment-regimen means were about 7.5% and 8.4% at 6 mg and 12 mg orforglipron at 72 weeks; Lilly's Foundayo materials highlight 12.4% at the highest dose among people who stayed on treatment. Cross-trial comparison is imperfect. Route convenience is a separate question from average percent loss.
Does Beema offer orforglipron or Foundayo?
No. Beema Health does not currently offer orforglipron. Live evaluation is for compounded semaglutide or compounded tirzepatide when a licensed provider finds them appropriate. Compounded semaglutide is not FDA-approved and is considered only when legally available and clinically appropriate. Is orforglipron oral semaglutide (Rybelsus or Wegovy pill)?
No. Orforglipron is a different small-molecule agonist. Oral semaglutide products have their own labels and meal-timing rules. Foundayo has been described by Lilly as usable without those food and water restrictions. They are not interchangeable.
Who might prefer a daily pill?
People who cannot or will not inject, people with needle phobia, or people whose weekly injection schedule fails in real life. Preference is not the same as predicted weight change. A licensed provider should also check other medicines, pregnancy plans, and gastrointestinal history.
Who might prefer weekly semaglutide?
People who want a once-weekly routine, who already tolerate injections, or whose clinician wants the longer semaglutide evidence file, including SELECT cardiovascular data for the 2.4 mg obesity dose. Again, that is a clinical conversation, not a ranking trophy.
Can I switch from semaglutide to orforglipron on my own?
No. Switching GLP-1-class medicines needs a clinician to manage overlapping receptor stimulation, residual delayed gastric emptying, and dose timing. Do not overlap leftover injections with a new daily pill.
