CagriSema combines semaglutide with an amylin analog

One weekly shot, two hormone pathways

CagriSema is Novo Nordisk's investigational once-weekly combination of semaglutide 2.4 mg and cagrilintide 2.4 mg. Semaglutide is the GLP-1 receptor agonist already used in approved products. Cagrilintide is a long-acting amylin analog. The hypothesis is that pairing satiety signals from two hormones can produce more weight change than semaglutide alone, at the cost of more gastrointestinal effects for some people.

REDEFINE 1: what the 68-week data showed

REDEFINE 1, published in the New England Journal of Medicine in 2025, enrolled adults without diabetes who had a BMI of 30 or higher, or 27 or higher with an obesity-related complication. People were randomized to the combination, semaglutide 2.4 mg alone, cagrilintide 2.4 mg alone, or placebo, all with lifestyle intervention, for 68 weeks.

On the treatment-policy estimand, mean percent weight change was 20.4% with cagrilintide-semaglutide versus 3.0% with placebo (difference about 17.3 percentage points). Participants on the combination were more likely than placebo to reach 5%, 20%, 25%, and 30% weight-loss thresholds. Gastrointestinal adverse events were very common. Those are trial results in a selected population, not a guarantee for any reader.

Novo Nordisk has also reported REDEFINE 2 in people with type 2 diabetes and overweight or obesity, and a REIMAGINE program focused on diabetes outcomes presented at the American Diabetes Association in 2026. Extra datasets inform regulators. They still do not make the combination a Beema product.

The 2026 FDA review is not an approval

On December 18, 2025, Novo Nordisk announced it had filed a New Drug Application for CagriSema for chronic weight management in adults with obesity or overweight plus a weight-related condition. Later company materials pointed to a possible FDA decision in the fourth quarter of 2026. No public PDUFA date is required for this article to be useful: the honest status in August 2026 is 'under review, not approved.'

If FDA approves the combination later, that still would not automatically add it to Beema's formulary. Beema's live offering is separate cash-pay compounded GLP-1-class options when a licensed provider decides they are appropriate.

Amylin plus GLP-1 is not a DIY stack

Amylin analogs and GLP-1 agonists both slow gastric emptying. Together they can increase nausea. REDEFINE 1's 80% gastrointestinal-event rate on combination therapy is the number to remember if you are tempted by social-media 'add an amylin peptide' advice. Unregulated cagrilintide sold as a research chemical is not the trial product.

Class issues that apply to semaglutide - gallbladder events, pancreatitis warning language, pregnancy planning, and the thyroid C-cell boxed warning on approved semaglutide obesity products - remain relevant to any future CagriSema label. Read a future Medication Guide if the product is approved. Do not infer safety from a headline percent.

How this differs from care you can start now

Approved semaglutide for chronic weight management (Wegovy) and approved tirzepatide for chronic weight management (Zepbound) are branded products with FDA labels. Beema does not sell those brands. Beema may offer compounded semaglutide or compounded tirzepatide after provider review. Compounded semaglutide is not FDA-approved and is considered only when legally available and clinically appropriate. Compounded tirzepatide is not FDA-approved and is considered only when legally available and clinically appropriate.

CagriSema is none of the above. It is an investigational combination. Wanting next-generation efficacy is reasonable. Substituting two gray-market vials is not equivalent to REDEFINE 1.

Questions to take to a licensed provider

Ask whether an available GLP-1-class medicine fits your history today. Ask how nausea would be managed if you start therapy. Ask whether you would be a candidate for a branded combination later if FDA approves one. Beema intake can begin those conversations for compounded options only. Completing intake never guarantees a prescription.
