SURMOUNT-4 is the core discontinuation evidence

SURMOUNT-4 enrolled adults with obesity, or overweight plus a comorbidity, without type 2 diabetes. Everyone received tirzepatide during a 36-week open-label lead-in. Those who met randomization criteria were then assigned to continue tirzepatide or receive placebo for 52 weeks. People who continued generally kept most of the weight off and even lost a bit more on average from week 36. People assigned to placebo regained a large share of the weight they had lost during the lead-in, though many still remained below their original baseline on average at week 88.

Practical steps before the last injection

Name the reason: intolerance, pregnancy, surgery, access, or a planned trial off medication. Review insulin or sulfonylureas. Plan follow-up within weeks, not a year later. Discuss protein intake and resistance training to support lean mass, without pretending exercise fully replaces tirzepatide.

If the stop is for elective surgery, distinguish a peri-operative hold from a permanent discontinuation and get anesthesia guidance because of delayed gastric emptying.

Safety while the drug is leaving

Gastrointestinal symptoms can continue for days. Hydration still matters. Pancreatitis and gallbladder warnings do not vanish the morning after the last pen. Seek care for severe abdominal pain.

Restarting later

In SURMOUNT-4, adults who switched from tirzepatide to placebo after 36 weeks of treatment regained a substantial amount of weight over the next 52 weeks compared with people who continued tirzepatide. Hunger often returns as the drug washes out. People taking Mounjaro for type 2 diabetes also risk worsening glucose if no replacement is arranged. Individual results vary.

How long does tirzepatide stay in the body?

Tirzepatide's half-life is about five days. Several half-lives pass before exposure is very low, so effects and side effects can persist for days to a few weeks after the last weekly injection. That is why missed-dose and surgery-hold instructions are time-based.

Do I need to taper Zepbound before stopping?

The Zepbound label does not mandate a formal taper. Some clinicians step down among labeled maintenance strengths (15 to 10 to 5 mg) before a full stop. That is optional and unproven as a way to prevent regain. Do not split pens to create unofficial taper doses.
